MTF Space-Available Care
Once you're Medicare-eligible, you can only use MTFs (military hospitals/clinics) if they have unused capacity.
Also known as: Military Treatment Facility, MTF, space-available
What is it?
At 65, retirees drop in MTF access priority. You're still TRICARE-eligible, but MTFs serve active duty and TRICARE Prime enrollees first. You may be seen only if there's space available — which often there isn't for primary care.
Why does it matter?
Many career military families assume they'll keep their on-base doctor at 65. That's usually not realistic. You need to find civilian Medicare providers before your IEP closes.
When you'll encounter it
Your existing MTF appointments may not be renewable starting the month you turn 65.
Impact on Medicare
MTF care at 65 doesn't bill Medicare — but you still need Part B to keep TFL for off-base care.
Impact on TRICARE For Life
TFL doesn't cover MTF care because MTFs don't bill TFL. Your civilian care is where TFL works.
Common misconceptions
- "I'll just keep my MTF doctor." — You can ask, but you're now the lowest priority. Plan to transition to civilian providers.
