Optional consideration

Should you also consider Medicare Advantage?

Once you understand Original Medicare + TFL, you can make an informed decision about whether a Medicare Advantage plan adds enough value to be worth it. This page is honest about both sides.

Make sure you've read "How OM + TFL work together" first

The honest summary

Medicare Advantage is a different way to receive your Medicare benefits, not an upgrade to Original Medicare.

Depending on the plan, Medicare Advantage may include supplemental benefits such as dental, vision, hearing, fitness programs, transportation, OTC allowances, and other benefits — availability varies by plan and ZIP code, and not every plan offers every benefit. Some retired military find Advantage adds real value through these benefits or Part B givebacks. Others prefer the freedom of Original Medicare + TFL with no network. There is no universally right answer — it depends on your doctors, your prescriptions, and your priorities.

Pros and cons, side by side

Where Medicare Advantage can win

  • Some plans include supplemental benefits like dental, vision, or hearing — major gaps in Medicare and TFL
  • Part B "giveback" on some plans reduces your monthly Social Security deduction
  • Some plans offer OTC, grocery, transportation, or fitness benefits — availability varies
  • All-in-one card and plan management for some people
  • Some plans coordinate with TFL, but not all

Where Medicare Advantage can lose

  • You're limited to the plan's provider network, not any Medicare doctor
  • Prior authorizations may be required for specialists, imaging, or procedures
  • Plan benefits, formularies, and networks change every year
  • Built-in Part D may overlap or interact with your TRICARE pharmacy benefit
  • If you travel widely or live overseas part-year, networks can complicate care

Benefits typically included

These are common, but never assume. Confirm exactly what each plan covers in your ZIP code.

Dental coverage

Cleanings, fillings, dentures and more, not covered by Original Medicare or TFL.

Vision care

Annual eye exams and allowances for glasses or contacts.

Hearing benefits

Hearing exams and significant allowances for hearing aids, a major gap in Medicare/TFL.

Prescription drugs

Most plans include Part D drug coverage built in (note: you already have TRICARE Pharmacy).

Fitness programs

SilverSneakers and similar gym memberships at no extra cost.

OTC & grocery cards

Many plans give monthly allowances for over-the-counter items or healthy food.

A simple decision framework

  1. 1

Are your current doctors Medicare-participating and not all in one Advantage network?

If yes and you value provider freedom, Original Medicare + TFL is likely your best fit.

  2. 2

Do you spend significant out-of-pocket on dental, vision, or hearing?

Advantage may meaningfully offset those costs through plan benefits.

  3. 3

Would a Part B giveback make a real difference to your monthly budget?

Some veteran-targeted plans reduce or partially refund your Part B premium.

  4. 4

Are you comfortable managing a yearly plan review during AEP (Oct 15 – Dec 7)?

Advantage plans change every year. Original Medicare + TFL doesn't.

Questions to ask before enrolling in any Medicare Advantage plan

This checklist is carrier-neutral. Use it on every plan you compare, regardless of who is offering it. Confirm answers in the plan's Summary of Benefits, Evidence of Coverage, and provider directory before you enroll.

Network

Are my current primary care doctor and specialists in this plan's network this year — and are they still accepting new Advantage patients?

Prior authorization

Which services (imaging, surgery, physical therapy, specialist visits, durable medical equipment) require prior authorization, and how long do approvals typically take?

Travel & part-year residence

What is covered if I travel out of state, snowbird, or live abroad part of the year? Is there a national PPO option or only HMO?

Drug formulary

If the plan includes Part D, are my prescriptions on the formulary, at what tier, and how does it interact with my TRICARE Pharmacy / Express Scripts coverage?

Specialist access

Do I need a referral from a primary care doctor to see specialists, and are the specialists I already use in network?

Coordination with TFL

Does this plan's billing accept TFL as secondary payer? How are my copays and coinsurance processed after the plan pays?

Annual maximums

What are the dental, vision, and hearing annual maximums, and what is the medical maximum out-of-pocket for the year?

Part B giveback

Is there a Part B premium reduction in my ZIP code, and how is it applied — directly to my Social Security check or as a separate credit?

Plan stability

Is the plan offered in my county next year? What changed from this year's benefits, and what is the carrier's track record of staying in this market?

Disenrollment

If this plan doesn't work out, when can I switch back to Original Medicare + TFL, and what are the rules for the MA Open Enrollment Period (Jan 1 – Mar 31)?
